Photo/Video Release Clause: *

I hereby authorize Variations Studio to publish photographs and videos taken of me and/or the minor children indicated on this form, and our names, for use in Variations studio printed publications and website as well as other marketing materials. I release Variations studio from any expectation of confidentiality for the indicated minor children and myself and attest that I am the parent or legal guardian of the children indicated on this form and that I have the authority to authorize Variations studio to use their photographs and videos and names. I acknowledge that since participation in the publications and websites produced by Variations studio is voluntary, neither the minor children nor I will receive financial compensation or personaliedcopies of the digital files. I further agree that participation in any publication and website produced by Variations studio confers no rights or ownership whatsoever. I release Variations studio, its staff, employees, faculty and affiliates from liability for any claims by or any third party in connect with my participation or the participation of the indicated minor children. By signing below I also hereby acknowledge that I am aware that performances by Variations studio will be videotaped strictly for private home viewing only. By signing this form, I am releasing the registered student’s image in order to participate within the performances choreographed and staged by the Variations staff/faculty and that photos of your child may also be used for marketing purposes.

Information Release Clause: *

Variations will never sell or give away client’s personal information to third party vendors. We are asked occasionally for classmate information for use as birthday invite lists and other personal event planning. Please select below if you allow Variations to release information to other classmates, upon request.

Absence Policy: *

Variations Dance Studio does not prorate due to absences and I understand any missed classes may be made up in an alternate time approved by Variations staff. By signing below, I indicated that I fully understand and agree that the owners/instructors of Variations shall be free of any liability of claims arising by reason of any injury or illness at Variations or any dance related activity with Variations. Permission is hereby granted to the staff of Variations to transport my child to a doctor or hospital in case of illness or injury when unable to locate the parent/guardian emergency name.

Medical Policy: *

I have read, understand and agree to abide by the policies of Variations. I hereby release, indemnify, and hold harmless Variations studio and their faculty from all liability or claims. I authorize the staff of Variations studio to give permission for medical treatment in the case of serious illness or injury.

Conduct Policy: *

Variations Dance Studio supports young people to build skills for life in an environment where integrity, kindness, and respect are taken seriously. Students are expected to act respectfully towards fellow students, visitors, faculty, and staff members. Parents are expected to be polite to one another as well. Any rude or aggressive behavior from parents will not be tolerated. Any use of offensive language towards any Variations employee will result in dismissal from the program without a refund. Parents are prohibited from disciplining a child other than their own. Any questionable student behavior needs to be directly reported to a Variations employee. Disrespectful behavior is not acceptable at any time. Variations may take disciplinary action or require the withdrawal of a student for failure to comply with the rules of conduct, or because of disregard of other major school regulations, or lack of parental cooperation.

Inclement Weather Policy: *

Variations follows our area schools in the event of inclement weather. If time allows, weather cancellations will be posted on our site, social media outlets, voicemail, and through our Remind text portal. Classes cancelled due to inclement weather will not be rescheduled or reimbursed. Students are encouraged to make up their classes by attending a class recommended by our staff, but are not required to do so.

Payment Policy: *

Tuition payments will be automatically drafted from a debit or credit card on the 1st day of each month, August through May. A $35 late fee will be assessed after the 10th for any declined cards that are unable to process for the full tuition amount. Please be sure that you have added and/or updated your debit or credit card information in your online account prior to August 1st. Accounts without a saved credit card on file by August 1st may forfeit their classes for our upcoming dance session. Any account that is over one month delinquent may result in the discontinuation of the student’s enrollment in classes at Variations. Only past due accounts will be billed by mail; All statements will be emailed to you on a monthly basis.

Student Withdrawal Clause: *

Variations requests in writing, all student withdrawals from class, two weeks prior to the beginning of the next month. Tuition will not be reimbursed for withdrawals mid-month or due to failure of notifying our front office prior to a new month beginning. Refunds cannot be given to students who withdraw from class after recital costumes have been ordered. In the event a student withdraws after this deadline, Variations will contact the client upon receiving the student’s costume to be picked it up at your convenience.
